Targeted Case Manager

The Mending Point program Targeted Case Manager (TCM) is highly motivated to make a difference in the community and committed to the well-being of their clients.
The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link, coordinate, and assist in implementing the appropriate resources.
They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with agency, licensing, and Medicaid standards.
This includes, but is not limited to:
assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs.
Caseloads may include diverse populations with a variety of clinical needs.

Essential Duties and Responsibilities Maintain contact with the Director of Community-Based Programs on a weekly basis (by phone and/or personal contact).
Demonstrates professionalism, cooperation, leadership, and a clear understanding of program purpose.
Completes client intakes, timely client reviews, and discharges for assigned case load.
Provides support to the Intensive Outpatient Program therapist and clients during Intensive Outpatient Program hours.
Assesses client and family strengths and needs utilizing Needs & Strengths Assessment.
Collaborates with client and other invested parties in the development and implementation of a strengths-based Service Plan.
Responsible for coordinating with the agency Insurance Specialist to obtain Prior Authorizations for TCM services.
Assist in maintaining thorough communication with clients, families, and involved agencies.
Maintain monthly billing documentation with up-to-date forms and dates obtaining appropriate signatures as needed.
Maintain up-to-date clinical records in agency's EMR system.
Maintain monthly productivity expectation as set forth by the Director of Community-Based Programs.
Provide liaison services between KyUMH programs, school personnel, courts, community agencies, employers, and other stake holders.
Collaborate with referral sources, mentors, family, and others involved in client's treatment as needed.
Conduct and document appropriate assessments.
Maintain current knowledge of community resources and refer clients as needed.
Assist with 24/7 crisis rotation/on-call service as outlined in agency and program policies.
Manage situations involving client risk and provide crisis services to de-escalate crisis situations and promote stability.
Ensure the competent and timely completion of required reports for KyUMH and all appropriate outside agencies.
Work with Community-Based Clinical Team to address barriers impacting clients' treatment.
Deliver services primarily in community-based setting or where necessary to meet client needs.
Consult with Director of Community-Based Programs, Community-Based Clinical Team, and other agency Clinical Directors, as needed.
Participate in all assigned meetings, staff development, and training as required.
Assist with supervision and teaching of the agency's interns and volunteers, as needed.
Transport clients to maintain family contact, court dates, family sessions as necessary and feasible.
All other duties as assigned by the Director of Community-Based Programs.
RETENTION BONUS OF $500 GIVEN AT 3 MOS/6 MOS/9 MOS FOR A TOTAL OF $1500.
00!Education & Professional Requirements:
Bachelor's Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor's degree.
Certification in the following Department For Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in Kentucky State Regulations and Statutes:
Ten (10) hour Community Support Associate Training Twelve (12) hour Targeted Case Management Core Training Six (6) hour Substance Use Disorder Targeted Case Management Training Six (6) hour Severe Emotional Disability Targeted Case Management Training Six (6) hour Severe Mental Illness Targeted Case Management Training These must be completed within three (3) months of date of hire Maintains required annual training as applicable per education level.
Additional training is required and provided for those without a degree or experience.
Job Specifications Must be at least 21 years of age.
Valid driver's license, reliable transportation, and proof of current auto insurance.
Valid certification in CPR and First Aid (can be provided upon hire).
Must submit to required background checks and pre-employment drug screening.
